Today, physicians, health care coaches, case managers, disease/population health management employers, and others involved in containing health care costs attest to the challenges faced in engaging individuals in their health.  Too often, clinically sound guidance is provided, patients seemingly understand what they need to do, and yet for many individuals behaviors remain unchanged over time despite significant risk to their health.

Without a systematic way to assess patient capabilities for self-management, health care professionals in medical management programs often apply a largely one-size-fits-all evidence-based approach. Yet as we know, patients bring differing levels of knowledge, skill and confidence to the task of managing their health. The Patient Activation Measure® addresses this challenge.
